Vraag een account aan

Vertegenwoordigt u een rederij, maritiem uitzendbureau of agentschap en is uw bedrijf in Nederland gevestigd? Dan kunt u op deze pagina met succes een account aanvragen voor deelname aan het Nederlandse COVID-19 vaccinatieprogramma voor zeevarenden. Vanaf 15 maart 2022 is het alleen nog mogelijk om voor zeevarenden (booster)vaccinatie afspraken te maken in de Nederlandse zeehavenregio's. Deze vaccinaties worden uitgevoerd door Arbo Unie / Port Health Centre.

Vul onderstaand formulier in als uw organisatie nog geen account heeft. In geval van een succesvolle aanvraag, ontvangt u in principe binnen 24 uur (weekenden uitgesloten) een bevestiging dat uw account is goedgekeurd. Daarna kunt u vaccinatie afpsraken voor zeevarenden inplannen volgens de procedure zoals gepubliceerd op www.kvnr.nl/vaccinatieprogramma. 

Apply for an account

Do you represent a ship manager / owner, maritime recruitment or placement agency, or ship agency and is your company based in the Netherlands? You will then be able to successfully apply for a verified account to participate in the Netherlands COVID-19 Vaccination Programme for seafarers. From 15 March 2022 it is only possible to schedule vaccination appointments in the Dutch sea port regions. These vaccinations will be executed by Arbo Unie / Port Health Centre. 

Complete the form below if your organization does not yet have a verified account. If your application is succesful, you will shortly receive a confirmation that your account has been verified and approved. Thereafter, you will be able to schedule vaccination appointments for seafarers in line with the procedure as published at www.kvnr.nl/vaccines4seafarers.
